Now everyone on the SRP-MIC-sponsored health plan may participate

Pre-register at your work site on Thursday (11/19) and Friday (11/20) and turn in your waivers. You will receive your shirts, meal vouchers and activity bracelets.

Need help or have questions at the event? Look for WellPath volunteers wearing ASU Gold WellPath shirts.

Map showing parking and location of events held in the Community Building, Gym, Parking Lot and Fields. Parking available for participants east of Longmore. Disabled and volunteer parking available east and north of the Community Building.

Print out your activity sheet or pick one up at the event. You and your family collect signatures from booths and activities. Turn in a completed sheet and earn 15 WellPath Points for each member of your family.

Each adult participant will sign and submit a liability waiver. Adults receive a meal voucher and an activity bracelet when submitted at registration. Children do not sign a waiver. Chldrens’ meal vouchers and activity bracelets will be provided to an adult who submits the liability waiver.
